Why Your Stress Score Ignores That You're on Medication

You started a new medication, or your doctor adjusted a dose, and your wearable's stress score suddenly looks worse. Nothing in your day changed. The app doesn't know that. It never asked.

What a stress algorithm can and can't see about you

A stress score is a pattern match over two signals: your heart rate and your heart rate variability (HRV), usually compared against your own recent baseline. It can see that your resting heart rate is higher than usual, or that your HRV dropped overnight. It cannot see why. It has no field for "started a new medication," no field for "dose changed last week," no field for anything happening in your body that isn't expressed as a heart rate or HRV number. The algorithm is reading an effect and guessing at a cause, and by default it guesses stress, because that's the label the product ships with.

How certain medications affect heart rate and HRV directly

Some medication classes act on the same physiological systems that produce your heart rate and HRV readings, which means they can shift the numbers a stress algorithm relies on without any change in how stressed you actually feel. This isn't a flaw in the drug or the sensor. It's two systems measuring the same underlying signal for different reasons.

Heart rate and HRV are both downstream of the balance between your sympathetic and parasympathetic nervous systems, the same balance that shifts when you're genuinely stressed. Certain medication categories are known to act on this balance directly. Stimulant-type medications, for example, are generally understood to push heart rate up and can compress HRV, the same direction stress usually pushes them. Beta blockers work in something closer to the opposite direction, generally lowering resting heart rate and altering HRV patterns, because that's the mechanism they're prescribed for. Neither example is medical guidance, and neither is a claim about how much either effect is worth in your own body. They're illustrations of a broader class: a medication that touches heart rate or the autonomic nervous system will show up in the same data a stress algorithm reads, whether or not anything stressful is happening.

Why the app has no way to separate medication effects from 'stress'

The app has no way to separate the two because it was never given the input it would need to do so. A stress algorithm is a closed system: heart rate and HRV go in, a score comes out. Unless a user (or a connected data source) explicitly tells it "I'm on medication X, started on this date," that variable simply doesn't exist inside the model.

This is a missing input problem, not a broken sensor. The sensor is doing exactly what it's built to do: measure heart rate and HRV accurately and compare them to a baseline. The scoring layer on top is doing exactly what it's built to do: flag a deviation from that baseline as "stress," because stress is the only label most consumer wearables are designed to output. Add a medication that shifts the same underlying numbers, and the algorithm has no competing hypothesis to weigh against "you're stressed." It was never built to hold that hypothesis in the first place.

What logging medication alongside metrics actually changes

Logging medication next to your stress and HRV readings doesn't fix the algorithm. It doesn't make the score smarter or clinically accurate. What it changes is what you can see when you look back: the dose or the start date sitting on the same timeline as the readings it might be influencing, instead of one living in a pharmacy record and the other in a wearable app that never talk to each other.

How to read a stress score once you know this limitation

Once you know a stress score has no channel for medication, read it as a description of your heart rate and HRV pattern, not a diagnosis of your mental or emotional state. If the score moved around the same time as a new medication or a dose change, treat that timing as relevant context, not proof of cause, since other things (sleep, illness, training load, alcohol) can shift the same numbers.

If you want more than a coincidence in timing, that's a conversation for whoever manages the prescription, not a pattern to diagnose yourself from the app. The wearable can tell you a number changed. It cannot tell you why, and it was never designed to. That part belongs to the people who actually know your health history.
